The high value form is used by the customer to declare the value of any exceptionally expensive items being tendered to the carrier for transport. The customer does not have to provide proof of value to designate an item as extraordinary value, but they will have to provide proof of value (appraisal, receipts, etc.) to file a claim in the event it is damaged. Customers should ensure that they purchase enough valuation to cover both high value items and the rest of the shipment items.
